Terms of Service

LERA operates an organized healthcare arrangement designed to deliver whole-body care through a coordinated structure.
Clinical services are provided exclusively by LERA Medical Informatics LLC ("LMI"), a healthcare services entity. Your clinical relationship as a member is with LMI.
LMI delivers health and wellness services through a team of licensed medical practitioners, whole-body health coaches, and mindful resilience coaches, all of whom are independent contractors of LMI.
LERA Health LLC ("LERA Health") provides the technology platform, administrative services, and operational support that enable care delivery.
LERA Nutraceuticals LLC ("LN") provides LERA's proprietary professional-grade supplements.
LMI, LERA Health and LN may be referred to collectively as "LERA" where appropriate; however, clinical services and medical decision-making are provided solely by LMI.
These Terms of Service ("Terms") govern your access to and use of the LERA website, platform, applications, and services (collectively, the "Services").
By accessing or using LERA, you agree to these Terms. If you do not agree, do not use the Services.
1. About LERA
LERA (also "we," "us," or "our") provides women's whole-body functional medicine services, which may include clinical care delivered by licensed medical practitioners, biomarker testing, education, and longitudinal health support.
LERA operates in a regulated healthcare environment. Certain services are subject to additional notices, consents, and policies.
2. Eligibility
You must be:
- at least 18 years old, and
- legally able to consent to medical care
By using LERA Services, you represent that you meet these requirements.
3. Scope of Services
Clinical Services
LERA provides clinical services through licensed medical practitioners in accordance with applicable state and federal laws.
Clinical services may include:
- review of health history and symptoms
- ordering and interpretation of diagnostic tests
- development of personalized care plans
- follow-up and longitudinal monitoring
Not all services are available in all states.
Testing Services
LERA may facilitate biomarker testing, including:
- blood testing through national laboratories
- stool and saliva testing via clinician-ordered or at-home kits
You agree to follow all instructions provided for sample collection and testing.
Education and Technology
Some content provided through LERA is educational and informational.
Technology, including rules-based systems and analytics, is used to support, not replace, clinical judgment.
4. Medical Disclaimer and Emergency Care
LMI does not provide emergency medical services.
If you believe you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency room.
LMI is not a replacement for your primary care provider or in-person specialty care. (See Medical Disclaimer for additional details.)
5. Privacy and Health Information
Your use of LERA is governed by:
- our HIPAA Notice of Privacy Practices (for protected health information), and
- our Privacy Policy (for non-clinical personal data)
By using the Services, you acknowledge receipt of these notices and agreement to the respective terms.
6. Consent to Care and Testing
By using LERA, you consent to:
- receiving clinical services from licensed medical practitioners
- ordering and processing of biomarker testing
- use of your information to support care delivery and personalization
Additional informed consent documents may be required before certain services are provided.
7. Pregnancy and Supplement Use
If you are pregnant, trying to become pregnant, or become pregnant while using LERA Services, you agree to notify your care team promptly.
Unless specifically reviewed and approved by a licensed healthcare provider, supplements should be discontinued during pregnancy.
8. User Responsibilities
You agree to:
- provide accurate and complete information
- update information as needed
- follow care and testing instructions
- use the Services only for lawful purposes
You are responsible for maintaining the confidentiality of your account credentials.
9. Fees, Payments, and Subscriptions
Fees for Services will be disclosed before purchase.
Unless otherwise stated:
- fees are non-refundable once services are initiated
- testing costs may be separate
- subscriptions renew automatically unless cancelled
Additional payment terms may apply and will be disclosed at checkout.
10. No Guarantees
Health outcomes vary.
LERA does not guarantee any specific results, diagnoses, cures, or outcomes.
11. Enterprise and Partner Programs
If you access LERA through an employer or partner:
- your employer does not receive individual health data
- reporting is aggregated and de-identified
- participation cannot be used for employment decisions
Your relationship with LERA is separate from your employment.
12. Intellectual Property
All content, software, trademarks, and materials provided through LERA are owned by or licensed to LERA.
You may not copy, modify, distribute, or reverse-engineer any part of the Services without written permission.
13. Prohibited Uses
You may not:
- misuse or interfere with the Services
- attempt unauthorized access
- use LERA for illegal purposes
- resell or commercialize Services without permission
14. Disclaimers
The Services are provided "as is" and "as available," except as required by law.
To the fullest extent permitted by law, LERA disclaims warranties of merchantability, fitness for a particular purpose, and non-infringement.
15. Limitation of Liability
To the extent permitted by law, LERA will not be liable for indirect, incidental, special, or consequential damages.
Total liability is limited to the amount you paid to LERA for Services during the 12 months preceding the claim.
16. Indemnification
You agree to indemnify and hold harmless LERA from claims arising out of:
- misuse of the Services
- violation of these Terms
- false or misleading information you provide
17. Dispute Resolution and Arbitration
Any disputes arising from these Terms or the Services will be resolved through binding arbitration, unless prohibited by law.
You waive the right to participate in class actions.
18. Changes to These Terms
We may update these Terms from time to time.
Continued use of the Services after changes become effective constitutes acceptance.
19. Governing Law
These Terms are governed by the laws of the United States and the State of Colorado, without regard to conflict-of-law principles.
