Before using LERA Health's services, please read this document carefully. By enrolling in LERA Health, you acknowledge that you have read, understood, and agree to this Informed Consent and Data Usage policy.
Nature of Services
LERA Health provides functional wellness services, including health assessments, at-home laboratory testing coordination, personalized health plan development, and educational resources. These services are designed to support your overall wellness and are not a replacement for emergency medical care, diagnosis, or treatment by a licensed physician.
Voluntary Participation
Your participation in LERA Health's program is entirely voluntary. You have the right to withdraw your consent and discontinue services at any time, subject to our membership terms and cancellation policy.
Health Information You Provide
As part of our services, you will be asked to provide detailed health history information, including information about your symptoms, medical history, medications, lifestyle habits, and health goals. You consent to:
- Completing health intake questionnaires
- Providing biological samples for laboratory analysis (blood, saliva, stool)
- Sharing relevant health records that may assist in personalizing your health plan
- Ongoing health tracking and check-ins through our platform
How Your Data Is Used
Your health information is used to:
- Develop and deliver your personalized health plan
- Monitor your progress and refine your recommendations over time
- Facilitate communication with practitioners involved in your care
- Conduct de-identified, aggregate research to improve our services and advance women's health
We will never sell your personal health information. De-identified research data (from which your identity has been removed) may be used internally or shared with academic or research partners for the purpose of advancing women's health science.
Laboratory Testing
LERA Health coordinates with CLIA-certified laboratories for your testing. By enrolling, you consent to:
- Collection and analysis of biological samples as part of your testing protocol
- Sharing your personal identifying information with laboratory partners solely for the purpose of processing and reporting your results
- Receiving and reviewing your laboratory results through the LERA Health platform
Risks and Limitations
You understand and acknowledge that:
- LERA Health's services are for wellness and educational purposes and do not constitute medical diagnosis or treatment
- Laboratory results may occasionally require follow-up with a licensed physician
- Individual results and outcomes will vary
- You should always consult with a qualified healthcare provider before making significant changes to your health routine
Right to Revoke Consent
You have the right to withdraw your consent at any time by contacting us at privacy@lerahealth.com. Please note that withdrawal of consent may limit our ability to provide certain services, and any information already used in connection with your care prior to withdrawal cannot be retroactively removed from completed analyses.
